Blue Cross Blue Shield Medicare Advantage plans are rated at about the industry average, getting an average rating of 3.96 stars out of 5 from the Centers for Medicare & Medicaid Services (CMS) for 2025, weighted by enrollment. For comparison, the average weighted star rating for all plans is 3.92.
Of Blue Cross Blue Shield’s rated plans, 48% are rated 4 stars and up. Of Blue Cross Blue Shield’s Medicare Advantage members who are in rated plans, 58% are in plans that are rated 4 stars or higher for the 2025 plan year.
CMS maintains its own database of star ratings on every Medicare Advantage and separate Medicare Part D plan, ranging from best (5 stars) to worst (1 star). The agency bases these ratings on plans’ quality of care and measurements of customer satisfaction, and ratings may change from year to year.

Although premiums vary, Blue Cross Blue Shield offers $0-premium Medicare Advantage prescription drug plans in 47 states and Puerto Rico. You'll also be responsible for paying your Medicare Part B premium, which is at least $174.70 per month in 2024 ($185 in 2025).
(Most people pay this standard amount, but if your income is above a certain threshold, you’ll pay the income-related monthly adjustment amount (IRMAA). Some Medicare Advantage plans cover part or all of your Part B premium.)

In its 2024 U.S. Medicare Advantage Study — the 10th it's done so far — J.D. Power measured member satisfaction with Medicare Advantage plans based on eight factors ranging from level of trust to how well a provider resolves problems or complaints. The study was fielded in the 10 biggest state Medicare markets, and here’s how Blue Cross Blue Shield companies ranked out of all major providers:
California: Blue Shield of California is 4th out of 8, Anthem Blue Cross is 6th out of 8.
Florida: Florida Blue is 4th out of 6.
Illinois: Blue Cross and Blue Shield of Illinois is 4th out of 5.

The National Committee for Quality Assurance, or NCQA, rates health insurance plans on a 5-point scale (with 5 being best) based on clinical quality, member satisfaction and results from the NCQA accreditation survey. The ratings highlight the results of care and what patients say about their care.
In its September 2024 ratings of Blue-branded plans, the NCQA awarded scores between 2.5 and 4.5.

The Blue Cross Blue Shield family of companies includes Elevance Health (formerly Anthem), the fourth-largest Medicare Advantage provider in the country among for-profit health plans. Elevance captured 6% of national market share for Medicare Advantage in 2024, according to a report from health care analytics firm The Chartis Group. Nonprofit Blue companies lost 0.1% of market share, accounting for 10.1% of the national market in 2024.
Blue Cross Blue Shield began as two companies — Blue Cross and Blue Shield — that offered affordable health care solutions in the early 1900s and started to come together in the 1940s. It was made official in 1982 when they formed the Blue Cross Blue Shield Association, which now includes 33 independent and locally operated companies across the country. Nationwide, it provides health care coverage to 115 million members in the U.S. and Puerto Rico.

It’s important to do your research before selecting a Medicare Advantage plan for yourself. Here are some questions to consider asking:
What are the plan’s costs? Do you understand what the plan’s premium, deductibles, copays and/or coinsurance will be? Can you afford them?
Is your doctor in-network? If you have a preferred medical provider or providers, make sure they participate in the plan’s network.
Are your prescriptions covered? If you’re on medication, it’s crucial to understand how the plan covers it. What tier are your prescription drugs on, and are there any coverage rules that apply to them?
Is there dental coverage? Does the plan offer routine coverage for vision, dental and hearing needs?
Are there extras? Does the plan offer any extra benefits, such as fitness memberships, transportation benefits or meal delivery?

The Medicare Advantage marketplace is concentrated among just a handful of companies; in many places, one insurer serves more than half of the market. NerdWallet reviewed 26 Medicare Advantage brands, with some insurers having more than one brand under their umbrella. (For example, Blue Cross Blue Shield includes the brands Anthem and Highmark.) At the national level, these reviews include nine of the 10 largest brands by enrollment. At the state level, our research includes at least one major insurer in 47 of the 49 states that offer Medicare Advantage plans, and includes the top two insurers in 35 of those states. (Alaska doesn't offer Medicare Advantage plans.) We also look at online search volume to identify regional and other notable players in the space.
